Forms

The following forms are for use by our church ministries. The forms are in Microsoft Word format and can be filled in online (preferred) or printed and filled out manually.

Ministry Activity Plan (MAP) - Use this form to request an event or activity to be added to the church calendar. Completed forms should be submitted to the Elders for approval.

Spending Approval - Use this form to request approval before purchasing any product or service. Unapproved expenses will not be reimbursed. Completed forms must be approved by the appropriate ministry chairperson and the finance committee secretary.

Check Request - Use this form to request reimbursement for a previously approved expense.
